You will require your contact lenses and/or glasses, a copy of your sight prescription.

You will need to bring a medical summary or login to your NHS App.
Dependent on your medical history and/or ECG result you may also require a Cardiologist’s report. This can be arranged at exta cost and will be required before final sign off.
If you require both ACU and FIM forms completing there will be a further £20 fee.

As per FIM guidance; with the exception of Trial any rider of 50 years and over must have an exercise tolerance (stress) electrocardiogram (ECG) performed, and the result must be favourable. In Trial, an exercise tolerance ECG is required for any rider of 50 years and over if there are known significant risk factors for or history of cardiac disease.
Sports Medical Licence Application FAQs
Who can complete my medical form?
Most sports require a GMC Registered Doctor, at Forza Sports Medicals you can be certain you will be looked after by a GMC registered and very experienced Doctor. All of our team still work as NHS GPs with the relevant GMC appraisal and revalidation.
Does my medical have to be completed by my usual GP?
No, unless specifically stated on the form as needing to be “usual GP or medical practitioner”. Please check the website for the regulatory body of your sport to check the requirement.
Do I get to keep a copy of my ECG?
Yes, sure, we can email you a copy, just ask at your appointment.
What if I fail the medical?
If your doctor identifies a concern, they may refer you for further specialist assessment. We do not offer treatment or diagnosis. Pease be aware that in some cases, a temporary licence restriction or suspension may be advised until the issue is resolved.
When should I book my medical?
As early as possible—ideally several weeks before you plan to apply or renew your licence. This allows time for any follow-up tests or paperwork.
